Empowering the patient or empowering the beneficiary?

Do we have to wait until we get sick?
In the first article of the “I and the Beneficiary and AI” series I said I would change my location frequently; sometimes I would sit next to the patient, sometimes I would enter the doctor’s clinic, sometimes with the nursing staff… and so on.
When I decided to start with the most important beneficiary (the patient), I asked myself this question.
Do I have to sit beside a hospital bed to see how AI could help the patient?
The truth is: no.
Because if we want the picture to be complete, we must start long before the hospital, from home, school, work, the gym… from everyday life itself, because
Patient empowerment does not begin after diagnosis; it starts much earlier.
The beneficiary is basically you, me, a son, father, mother, a family member and a member of society. If we wait for the word “patient” to care about our health, we are usually starting too late.
Therefore, patient empowerment does not have to be in a clinic or beside a hospital bed. It can be:
At home, next to a mother trying to teach her child a healthy habit.
Or in a club or gym.
Next to a healthy person who wants to reduce the chances of developing a chronic disease.
Or at school, next to a child learning how to protect themselves before getting ill.
What does a “empowered patient” mean?
Patient empowerment has many definitions; here I will use one from the Saudi Center for Patient Safety.
It is a process by which individuals, families, and communities acquire the ability, knowledge, and confidence to take actions that protect and improve their health, and to participate in designing and managing their own care.
Empowerment occurs at three levels:
The individual, the community, and the health system.
If I condense patient empowerment into six tasks, they are:
Understand, ask, choose, participate, act, and know when to request help.
Empowerment is not about the patient diagnosing themselves, nor being solely responsible for everything, nor handing them a complex medical report and saying: feel free to join the decision!
True empowerment
The information is understandable,
the choices are clear,
the person’s voice is heard,
and the decision is made in partnership with the health team.
So… where is the
AI
in the patient empowerment journey and what value does it add?

AI is present at every stage from prevention to coexistence.

First stage: prevention and risk reduction

When we are healthy… we understand risks and build habits
AI could
AI
help each person translate general guidelines into a simple plan suited to their age, routine, and goal: organizing sleep, increasing activity, improving diet, preparing for appropriate preventive screenings, or tracking a small healthy habit instead of big decisions we start enthusiastically and abandon after two days.
Empowerment can start in childhood.
Imagine a mother, instead of repeating every night: ‘Don’t eat too many sweets’ or ‘Wash your hands’, using AI tools to turn the message into an illustrated story or short video, with the child himself as the hero, by name, image, or favorite character.
Each week a new adventure builds a healthy habit or helps him quit a harmful one. Then the child shares the story with friends at school or the club, extending the impact from one child to a group of children.
This idea resembles the Saudi Center for Patient Safety’s “Safety Friends” program, which used fun, interactive sessions to teach children concepts of patient safety, medication safety, and infection control. According to the center, over a million pupils in more than 7,000 schools benefited, showing how a simple child-appropriate story can extend its impact from home to school and community.

Second stage: early detection and diagnosis
When a symptom appears…
We organize notes and understand when to monitor and when to seek help
A person could log symptoms chronologically, and the
AI
helps them organize them and prepare questions to ask the doctor
It can also explain the meaning of ‘warning signs’ found in a reliable medical source.
It could recommend the best hospitals or doctors in their area
The value here is not
that AI
AI
telling them: what do you have;
The value is that it helps them reach care while being better able to describe what happened and not forget important information.
But if there are severe, sudden symptoms or a possible emergency, the role of the
AI
stops immediately.
There is no chat or waiting for a reply here; it’s direct communication with emergency services or the provider.

Third stage: treatment in health-care settings

Before the visit… turn anxiety into preparedness
Before the appointment, the patient or a family member can ask the
AI
to organize the medication list, allergies, past surgeries, and to turn unstructured notes and concerns into a short summary, suggesting important questions to ask the doctor and preparing them so they aren’t forgotten, such as:
What is my diagnosis?
What options are available?
What are the benefits and risks of each option?
What signs, if they appear, would be red flags that I need to return to you quickly? … etc.
In this case the
AI
does not take the doctor’s place; on the contrary, it improves the quality of time between patient and doctor, because the patient arrives prepared and the doctor understands their details and notes more clearly, and the consultation starts from a higher level of understanding.

After tests or diagnosis… we understand before deciding

Medical reports are full of terminology that can intimidate anyone even when the result is simple.
AI
AI
can rewrite the report in plain language, explain the meaning of terms,
and can turn the treatment plan into daily steps, or compare options in a table showing benefits, risks, and each choice’s impact on the person’s life, values, and priorities.
This facilitates shared decision-making: the doctor provides evidence and clinical expertise, the patient brings their criteria and perspective on quality of life based on their values, abilities, and circumstances.
AI
AI
can prepare the discussion space, but it does not make decisions in place of either party.

Fourth stage: self-care and long-term therapeutic management

During treatment and coexistence… we monitor, learn, and share
In chronic diseases specifically, most of the journey occurs outside the hospital. Here AI can
AI
help the individual and their family coordinate care, record side-effects and readings, note what has changed in their quality of life at home, school, or daily activities, and detect any recurring pattern that needs discussion with the doctor; it assists in summarizing key observations between visits, and transforms lengthy instructions into a simple daily card, video, or infographic suited to each family member’s age, literacy level, and preferences.
However, the summary must be reviewed, data must be protected, and any start, stop, or dosage change must remain the doctor’s decision.
From empowering an individual… to empowering a community and health system
AI
can produce
, health content in various languages and formats,
summarize frequently asked questions, and turn family experiences into clear needs
can analyze patient experience surveys about their journeys so health institutions know where improvement opportunities lie.
and can also build solutions and systems that help address recurring challenges and provide support
but
we must hear the beneficiary’s voice before building the solution, during its design, and after its use.
because the best app, in a programmer’s or doctor’s view, may not solve the toughest moment in a patient’s life at all.

Research says patients are indeed not waiting for AI to be used

in a published research paper titled
Patients are not waiting for permission: the rise of the AI-empowered patient,
People have already begun using AI tools for health tasks outside the health system, so the realistic question is no longer: should we allow it? Rather: how do we make usage safer, and design research and services with patients and care providers?
And in a case study published in
Journal of Participatory Medicine,
one family used AI as a learning partner: to understand a complex medical report, search for diagnoses and options, prepare for visits, summarize information and share it with the care team. The family felt greater understanding and confidence, but researchers emphasized the need to review outputs and protect privacy.
There is also a systematic review covering 26 studies
that found decision-support tools powered by
AI
showed opportunities to improve understanding, participation, and adherence
. However they also showed
risks such as bias, outdated information, anxiety, privacy concerns, and digital exclusion.
In short, AI can add real value to the patient empowerment journey, but there are also risks
new ones that could arise if we use it without clear boundaries
So what is the solution to capture AI’s benefits in the empowerment journey while avoiding its risks???
We apply a traffic-light colour code: green, yellow, red
Green use, generally appropriate:
explaining information from a reliable source in simpler language, organizing appointment questions, summarizing medical notes (after removing personal data), preparing a healthy-habit plan, or turning an educational message into an age-appropriate story or image.
Yellow use, requires human confirmation:
interpreting a lab result or report, comparing treatments, understanding diagnostic likelihood, or linking multiple symptoms.
Use the
AI
to understand and prepare questions, then verify the information with an official source and a specialist doctor who knows your full case.
Red use, stop here:
final diagnosis,
delaying going to emergency,
starting, stopping, or changing medication dosage,
or uploading sensitive identifying data.
In this area the
AI
does not have context, examination, or clinical responsibility
and keep five safety habits in every interaction with
AI
remove names, identities, and contact numbers from documents
request the source of the information and its date
compare with official websites
tell the doctor you used
AI
and what you understood from it, so they can confirm or correct the information

… “Secure lifelong care”
On 17 September, the 2026 World Patient Safety Day will be celebrated,
and this year’s theme
secure care for non-communicable diseases
under the slogan “Secure lifelong care”. One of the campaign’s best messages is that people with non-communicable diseases should be seen as partners, not merely patients, and that safety exists in daily life, not only within health facilities.
That summarizes the whole article:
Patient safety and empowerment is not a moment inside the hospital. It is a journey that starts with prevention, passes through diagnosis and treatment, and continues every day at home, school, work, and the gym.
So can I finish the article like this without telling my story about patient empowerment and AI???
But that’s the point!!! The series is even called “I and the Beneficiary and AI”
So you all need to hear my story

When I started studying and learning about AI, I got the idea for an application

“Sugar Hero”

Instead of the child with diabetes seeing themselves constantly as a patient needing measurements, injections, and restrictions, why not see them as the hero of their journey? Why not present information and support in a way they understand, that empowers them and their family in the illness journey
I turned the idea with a team of AI friends (actually more like siblings, I’ll write about them in another article) into a small prototype we can build on
My dream is not just an app that gives information.
My dream is a comprehensive empowerment tool
that turns medical reports and instructions into heroic stories that the child understands and becomes the hero of
The family helps record and organize important information in a structured way that makes it easy to review and share with the doctor
It provides a safe space to connect with specialists and with families on the same journey, to exchange experiences and reduce the feeling that each family is fighting alone
But
As long as the goal is (empowering the child and their family), it means we cannot build the complete product and then tell them, “go ahead and use it”
We must first ask them: what is the hardest moment in your journey? When do you feel afraid? What information does not reach you?
What does the child reject? And what makes them feel like a true hero?
The prototype is not the end of the idea, it is the start of a dialogue
From a quality perspective, the success of “Sugar Hero” will not be measured by the number of times the app is opened, it will be measured by the most important questions:
Has the child understood their condition better?
Has the family’s confidence increased?
Has communication with the doctor improved?
Has adherence to the plan become easier?
Has the child’s fear and sense of being different decreased?
And have we added value without introducing a new risk?

And in the end… the
AI
is not what empowers the patient
the
just a tool
What truly empowers the patient is that we use the tool to turn information into understanding, understanding into a question, the question into a shared decision, and the decision into a behavior the person can sustain
I don’t want
AI
to tell the patient what to do instead of the doctor. I want it to help them understand, prepare, speak, participate, and live their health journey with greater confidence
and I don’t want technology to become a new burden on the person; I want it to bring care closer to their life, in their language, and at the time they need it
